Pet Declaration Instructions
Step 1: Collect Paperwork
Gather any existing documentation, such as adoption papers, digital photos, or veterinarian paperwork.
Step 2: Complete Form
Complete all sections of the Declaration self-service form and check the agreement acknowledgement.
Step 3: Print & Sign
Print your completed Declaration. Then immediately sign and date the document to make it official.
Step 4: Store Safely
Store the Declaration and any supporting records in a safe yet easily accessible place for when needed.
